No, we do not take insurance. Rebalance is a direct-pay for service clinic and payment is required at the time of service. Although we do not bill your insurance we will provide you with a superbill, which you can submit to some non-Medicare insurance plans for reimbursement. If you have questions about your insurance plan's reimbursement policy for out of network care, please call them for more information.

First Visit Overview:
The therapist will arrive fully equipped, including an examination table, for your initial evaluation. Together, you'll discuss your goals, form a treatment plan, and conduct a tailored exam based on your symptoms. Before you leave, you'll receive a (manageable) set of exercises to work on until your next visit.
Subsequent Treatment Visits:
As you progress, subsequent visits (usually 60 minutes) will be more focused on targeted treatment. Your therapist will bring a variety of equipment to enhance your recovery experience, making each session both effective and enjoyable.

Begin with a brief phone consultation. This will help us determine whether PT is right for you.
Forms Sent to You:
Following the consultation, we'll promptly send you the necessary forms.
Early Form Submission:
To streamline the in-person experience, kindly fill out these forms before your first visit. This ensures a smoother process, giving us more time to delve into your story, perform a physical exam, and provide tailored treatment.
Initial evaluation: This is your first visit, designed to assess your needs if we haven't seen you before or if it's been a while. It's also crucial for addressing entirely new concerns.
60 minute follow-up treatment: After the initial evaluation, this becomes your standard visit for routine follow-up treatments
30 minute express follow-up treatment: Reserved for special circumstances when a full treatment session is not necessary. These are at the therapist's discretion.
30 minute virtual appointment: This visit type is for when those in-person sessions just can't happen (illness, snow storm etc.).Your therapist may recommend this if it's beneficial for your progress.
Monthly check-in: Being discharged from PT can be a little overwhelming. Monthly check-ins are valuable for maintaining consistency toward long-term goals. These sessions ensure your exercises are still effective, offering motivation for your home exercise program as someone monitors your progress.
